Collaborative - System of Care Continued
Incorporated in this model is an eReferral system routing the referrals and demographic information to the appropriate agency/agencies based on their programs providing those services. Email notifications are automatically submitted to the assigned coordinator(s) and workflow task inbox to be reviewed, scheduled and worked. Referrals may be self-referred and/or network agency and/or outside the network with tracking and escalation processes.
Based on the agency’s programs and services, the client/participant EHR may encompass referrals, intakes, EDIF, assessments, CANS/FANS/ANSA/ CRAFFT/CASII tools, evaluations, service plans, flex funds, safety plans, incident reports, wraparound team meeting services, CFT planning notes, crisis plan, progress notes, measurements, attendance logs, wrap satisfaction survey, closure, etc. The Pace+ Community-based SOC model satisfies HIPAA, FERPA, and CFR-42 privacy guidelines and manages the sharing of client-specific information between agencies without risk of inadvertently passing Protected Health Information (PHI). Multiple agencies may serve the same client/student/participant where the information is segregated and secured under each agency. One of the greatest built-in tools is the sharing and releasing of consented information by the client’s authorization disclosure process. Release of specific information and to which agencies is granted by the client/student or guardian and can be revoked at anytime. Agency requesting information are reviewed and validated through the workflow disclosure mechanism to keep the agency holder anonymous should they decline. The workflow engine is an integral part of the solution, enabling agencies to streamline and automate processes to increase productivity and eliminate client fall-out. Supervisors/evaluators/counselors/caseworkers/coordinators/clinicians/facilitator/etc can receive notifications, reminders, staging/request approvals, escalations based on their roles and rules set within the agency.
The solution was designed to manage many types of care such as prevention/intervention/outreach planning services, culturally & evidence-based practice/competent services, wraparound services, faith & community-based involvement, SED, Counseling/Mentoring, education/school programs & attendance, family/youth/child-guided care/support, emergency aid, child abuse prevention, substance abuse, etc. Screening tools and eligibility validation rules are incorporated to facilitate proper access and treatment within the program.
