COLLABORATIVE

PACE+ supports collaboration for Health Information Exchange

Internet Connectivity: Because PACE+ operates over the Internet with role-based, tiered security, access to the system can be safely provided to whomever the organization desires. Not only can multiple agency clinicians participate, but outside organizations can have access to a client record, like teachers, probation officers, primary care staff, etc. In this manner PACE+ offers collaborative health information exchange among many providers. Virtual clinical team webinars can enable more efficient collaboration and participation in treatment planning and progress review. Should an organization desire, access to parts of the record can even be provided to clients. Thus the electronic health record can serve as a personal health record as well.

Web-Based Portability: Access to the PACE+ system is available wherever the Internet is connected; in other words, virtually everywhere. This is a plus for staff members working outside the office or regular office hours. Controlled by login and password security, users can record notes or complete an assessment where they happen instead of waiting to return to the office.

Community-based Network System of Care: The PACE+ Community-based Network System of Care (SOC) model empowers communities with a network of agencies (i.e. Schools/Independent School Districts, Child Welfare, Juvenile Justice, CPS, City/County/State Health agencies, Mental Health providers, Dental providers) to collaborate and manage clients separately or with others under one solution. It is a cohesive web-based [online] data management and reporting system for the agencies’ users at multiple sites. The SOC model customizes its processing, validation, data-gathering and client management to fit all agencies’ needs and encompasses key integrated Child/Adult Mental Health, Social Services, Dental Initiative/Grant components to manage, track, measure, report, and validate client and participant programs and services. The model represents true Health Information Exchange and development of the client’s medical home.

Electronic Signature (e-Signature): Users may sign documents by uploading their electronic signature under their password-prompted profile or with a signature pad. Multiple signatures [including the client’s] are allowed on the same document with the ability to sign in intervals. PACE+ is compliant with most of the Topaz SystemsTM capture pads.
